MD Diagnos LLC is required by law to maintain the privacy of your protected health information (PHI), provide you with this notice, and follow the terms currently in effect.
How we may use and disclose PHI
For treatment, payment and our own healthcare operations; to communicate with clinicians involved in your care; and as required by law or to prevent a serious threat to health or safety.
Your rights
You have the right to inspect and copy your records; to request an amendment; to request restrictions on certain uses; to request confidential communications; to receive an accounting of disclosures; and to receive a paper copy of this notice on request.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our privacy officer at admin@mddiagnos.com or with the U.S. Department of Health and Human Services. We will not retaliate for filing a complaint.
