We collect information you give us directly — your name, contact details, date of birth, insurance information (if you submit a superbill request), health history and any laboratory or imaging records you ask us to review. - To schedule and deliver the diagnostic services you request.
- To produce and deliver your written report.
- To coordinate care with your primary clinician, with your written consent.
- To improve our website and respond to requests you send us.
- To comply with our legal and regulatory obligations.
4. How we protect it
Patient health information is stored in HIPAA-aligned, encrypted systems located in the United States. Access is restricted by role, audited monthly, and revoked the same day staff transitions occur. Reports are delivered by encrypted email or hand-delivered.
5. When we share it
We never sell your information. We share it only: (a) with clinicians or laboratories you authorize in writing; (b) with vendors who help us deliver our services under a Business Associate Agreement; (c) when required by law, court order or to protect health or safety.
6. Your rights
You may request a copy of the information we hold about you, request a correction, ask us to delete information that we are not legally required to keep, or withdraw consent at any time. Email admin@mddiagnos.com and we will respond within thirty days.

8. Children
